Insurance and Prescription Referral
More and more insurance companies are covering Medical Massage treatments, and our practice now offers insurance billing.
If you believe that your insurance carrier covers Medical Massage treatments, please download our Prescription Referral form. Once we have your Prescription Referral, along with a copy of your insurance card, we will contact your insurance provider to verify coverage.
